<div><p>Health & Wellbeing Practitioner</p><p>Location: Cardiff</p><p>Salary: £26,787 per annum</p><p>Vacancy Type: Permanent</p><h3>About The Role</h3><p>Were looking for a passionate and proactive Health and Wellbeing Practitioner to join our team at HMP Cardiff on a full-time basis (37.5 hours per week). This is an opportunity to support individuals on their journey towards positive change, helping them improve their health, wellbeing, and prospects for a successful return to the community.</p><p>You’ll manage a varied caseload, deliver tailored interventions, and work collaboratively with service users to develop meaningful care and release plans that support long‑term recovery and wellbeing.</p><h3>What You’ll Be Doing</h3><ul><li>Managing a caseload of service users and coordinating their treatment journeys</li><li>Conducting assessments and developing person‑centred care plans</li><li>Delivering structured 1:1 interventions and facilitating group sessions</li><li>Providing harm reduction advice and wellbeing support</li><li>Supporting release planning and through‑care arrangements for individuals returning to the community</li><li>Carrying out second signatory duties and supporting clinical services</li><li>Working closely with multidisciplinary teams and partner agencies</li><li>Maintaining accurate case records and performance data to meet service targets</li></ul><h3>What We’re Looking For</h3><ul><li>Experience supporting individuals with complex health, wellbeing, substance use, or related needs</li><li>Strong assessment, case management, and communication skills</li><li>Ability to build positive relationships and motivate change</li><li>Excellent organisational skills and attention to detail</li><li>Confidence working in a fast‑paced, target‑driven environment</li><li>A collaborative and solution‑focused approach</li></ul><p>This is more than a job, its a chance to play a vital role in rehabilitation, recovery, and reducing reoffending. These checks can take up to 5 months to complete.
