Job Summary

Care Coordinator (Learning Disabilities) (£16.10p/h)

In Caring Hands specialises in person-centred care of young adults with profound multiple learning disabilities and complex health needs in our new respite home in St Austell town and in the surrounding community.

Responsibilities and Duties

In the caring profession, the skills you need can be taught, and we make a commitment to train all our staff, and continue developing their skills throughout the time they work with us. Compassion, kindness and empathy can't be taught, together with the ability to get along with people, even if they are elderly, disabled, grumpy, sick or just having a bad day. If you have these things, along with the basic skills to fulfil the needs of this position, we can help you learn the rest.

As a Care Coordinator you will be responsible for assisting with rota changes as necessary, updating care plans, spot checks, liaising with staff, service users and their families/advocates as appropriate.

This position is full-time in our location in south central Cornwall. You are expected to work alternate weekends as part of a rota system with your colleagues.

We welcome applications from those with at least one year experience of working with individuals with Learning Disabilities.

We would like to be very clear that this role involves at least half of your time delivering care.

Essential requirements

· NVQ Level 3 in Health and Social Care or equivalent

· Experience of working in a caring environment supporting individuals with a learning disability for at least 12 months

· Ability for flexible working including evenings and weekends

· Good communication skills, both written and verbal

· Full driving licence with access to own vehicle

· Caring and empathetic nature

· Strong ability in team working and organisational skills

· The ability to work positively, with good humour

· Willingness to learn to improve service delivery

· Take responsibility for actions and plan effectively to deliver results

· Experience of supervising and managing a team.

Desirable

· First Aid certificate

· The ability to develop knowledge as required by the role

· Previous experience supporting individuals with PMLD.

· Knowledge around Epilepsy, Enteral Feeding and complex health needs.

· NVQ Level 4 in Health and Social Care or equivalent.
