An exciting opportunity has arisen at Chelsea and Westminster NHS Foundation Trust to be part of a new dynamic Acute Frailty Team. We are recruiting two cross-site ACPs, six Band 7 Frailty Practitioners, and four Band 6 Frailty Practitioners. The Band 7 and Band 6 roles are site-specific.
All candidates must have a passion for frailty, be proactive, and ideally have experience caring for frail, elderly patients. Working as part of a multidisciplinary team, the post holders will lead and support the development of a new service tailored to our community's needs. This partnership requires innovative thinkers committed to providing high-quality, safe care at the right place and time. The post holders will develop holistic, integrated, and personalized care plans to ensure positive outcomes for patients. They will be essential in maintaining service consistency and fostering communication across secondary and community care sectors.
Main duties of the job
* Participate actively in multidisciplinary care, including frailty screening, comprehensive geriatric assessments (CGA), Same Day Emergency Care (SDEC), and virtual ward services.
* Provide skilled assessment, planning, delivery, and evaluation of frailty conditions within established guidelines and protocols.
* Order, perform, and interpret relevant clinical tests and investigations.
* Offer expert frailty advice and guidance to medical, nursing, and other multidisciplinary team colleagues.
* Recognize the importance of the Specialist Palliative Care (SPC) team as a key partner, considering Advance Care Planning and the use of tools like the Urgent Care Plan (UCP) and London Care Record (Cerner) as patient-centered, admission-avoidance strategies.
* Confidently complete physiotherapy and occupational therapy assessments, and formulate and implement discharge plans for elderly frail patients presenting to A&E, Acute Medical Units, Acute Frailty Units, and SDEC.

Job responsibilities
* Manage a caseload of patients within the Acute Frailty Service, including within the Emergency Department and SDEC, working autonomously and as part of the multidisciplinary team.
* Conduct comprehensive geriatric assessments, including ordering, performing, and interpreting clinical examinations under supervision, and providing advice and education to patients and the wider team.
* Be aware of professional limitations, act within your scope of practice, and use professional judgment in managing complex situations.
* Empower and advocate for patients in decision-making regarding their care.
* Complete physiotherapy and occupational therapy assessments independently and develop discharge plans for elderly frail patients presenting to various hospital units.
