Telford and Wrekin Community Mental Health Service provides assessment and evidence-based interventions for people presenting with complex mental health difficulties significantly impacting their daily lives, alongside our partner organisations.
We are looking for a practitioner to join our team to work alongside primary care practitioners and wider partners to support triage and assessment of those referred into our service. You will need to encourage effective engagement with this cohort, utilising excellent communication skills, whilst maintaining compassion and hope through every contact. We also aim to incorporate a trauma-informed psychological approach to increase independence and improve quality of life. There is an emphasis on team-based multidisciplinary decision-making, creating a supportive environment for staff. We value a culture of active service and personal development for all staff.
The role involves:
1. Supporting triage and assessments of individuals referred into the service alongside a multidisciplinary team including community mental health nurses, psychologists, consultant psychiatrists, occupational therapists, peer support workers, healthcare support workers, and housing and financial advisors.
2. Undertaking assessments individually, with the team, or with partners to establish the person's goals, needs, risks, and strengths, and generating a formulation-based plan of care with the individual and their support network.
3. Providing clinical advice and guidance to patients, families, and partner services as a point of contact.
4. Offering brief interventions where appropriate to support recovery and potentially prevent long-term involvement.
5. Participating in the local mental health alliance network, attending weekly meetings to collaborate on supporting individuals with complex needs, risks, and engagement challenges.
