Hepatico-pancreato-biliary (HPB) Oncology Clinical Nurse Specialist

An exciting opportunity has arisen within the Cancer Services directorate, for a HPB Cancer Clinical Nurse Specialist (CNS) to work within the HPB Cancer CNS team.

We are looking for a dynamic flexible individual who has excellent communication skills and experience within the field of HPB Surgery and/ or Oncology or relevant cancer experience across the Peninsula. This is an exciting opportunity for a proactive, self-directed individual to develop their skills in managing and caring for the complex and diverse needs of the HPB Oncology patient and their families across the cancer pathway, providing expert advice and coordinated support to patients with a diagnosis of pancreatic, biliary, gall bladder, small bowel, colorectal liver metastases and neuroendocrine cancers. We are looking for an experienced Nurse who is committed to delivering high standards of holistic care to all patients, working with and towards cancer and peer review standards and responding to service development needs.

Main duties of the job

***Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***

Assess, plan implement and evaluate expert holistic nursing care and provide support to patients within the HPB service and their carers. The post holder will have continuing responsibility for the co-ordination and organisation of individual treatment programmes throughout the HPB pathway/ illness trajectory, and work to develop communication across divisional and organisational boundaries.

Provide specialist nursing advice and psychological support to patients, carers, and health care professionals.

Acts as a resource, and contributes to the education and training of other health care professionals as required.

Contributes to service development, audit and research projects and helps develops educational packages according to need of care.

Job responsibilities

Clinical

* Act as keyworker to patients and relatives whilst undergoing cancer treatment at Plymouth Hospitals NHS Trust.

Manages his/her own caseload of patients referred from MDT and Cancer Nurse Specialists in tertiary centres. Works closely with other members of the Multidisciplinary team.

Provide expert advice to patients undergoing treatment for HPB cancer

Will work as part of the HPB Cancer Nurse Specialist Team, providing cross cover for other members of the Team as required to ensure continuity of cancer care is provided to all patients across the pathway.

* Communicates highly sensitive and emotive condition related information to patients, relatives. E.g. terminal illness, bereavement
* Assess new patients in the clinical area, identifying their psychological, physiological, emotional spiritual and cultural needs in relation to their disease and proposed treatment, providing a pathway of nursing care for the patient
* Act as patient advocate when relevant and be able to educate on all the treatment options to enable a patient to make an informed choice, negotiating with the medical team to agree a pathway of care
* Provide continuity of care throughout the patient journey and liaising across specialised areas ensuring that care is seamless between tertiary centres.
* Contribute to the development of specialised programmes of care and provide specialised advice, written and oral information to patients, relatives.
* Provide clinical advice and support to health care professionals within and across organisational boundaries
* To formulate individualised patient treatment care programmes in conjunction with the medical/nursing team considering
o Each patient is placed in the correct treatment protocol
o Investigations and treatments are organized as needed
* Act as a role model
* To provide support for distressed patients / carers and bereaved relatives and identifying patients at risk and referring to appropriate agencies

Initiates changes in management of care

* Participate in case conferences in relation to future management or discharge arrangements
* Maintain accurate patient records and ensure all relevant information is documented in medical and nursing notes
* In collaboration with multidisciplinary team members assess the needs of patients, families and carers and provide specialist advice.
* Assist with the development of a specialised nursing service provision
* Assist with the development of the Rapid Access Jaundice pathway and the care of patients going through this pathway.

Person Specification


Knowledge and Experience

* oProven evidence of Continuous personal and professional development
* oSignificant demonstrable and relevant post registration experience, at Band 5 or above, in cancer, palliative care or relevant speciality
* oProven knowledge in relevant speciality
* oDemonstrable experience of audit and research leadership and demonstrable management experience


Qualifications

* oRegistered General Nurse
* oFirst level degree in related subject or undertaking a Degree Pathway or the equivalent experience.
* oPost-basic qualification in cancer/palliative care or equivalent relevant subject or evidence of working towards / willingness to undertake within first year .
* oResearch qualification
