PCN Paramedic

The Paramedic will work autonomously within the Rural Warwickshire North PCN as part of a multidisciplinary team to provide advanced clinical assessment, triage, treatment, and care to patients in a primary care setting. This role includes managing undifferentiated presentations, supporting long-term condition management, and reducing demand on GPs through skilled clinical input and urgent care provision.
Main duties of the job
The Paramedic will be responsible for providingautonomous, advanced clinical care to patients presenting with undifferentiatedconditions. This includes conducting clinical assessments, managing acute andchronic conditions, and undertaking home visits when required. The postholderwill work independently within their scope of practice and support the PCNsgoals of delivering integrated, patient-centred care.
The role will contribute to improving capacity,reducing health inequalities, supporting frailty and chronic diseasemanagement, and enhancing same-day access to care. In addition, the Paramedicwill engage in service development, clinical audits, and population healthinitiatives to support the wider objectives of the Primary Care Network.

We are a Primary Care Network (PCN) of five practices working together with a range of local providers, including primary care, community services, social care, and the voluntary sector, to offer more personalized, coordinated health and social care to their local populations.
Our PCN comprises of 35k+ patients over the five practices, making us large enough to provide resilience and support the development of integrated teams.

Job responsibilities
1. Key Responsibilities:
* Conduct comprehensive clinical assessments during home visits to patients with varying medical needs.
* Provide immediate care, treatment, and advice within the scope of paramedic practice.
* Make independent clinical decisions based on assessment findings, patient history, and clinical guidelines.
* Manage patient safety and welfare during lone working, adhering to all relevant policies and procedures.
* Collaborate with multi-disciplinary teams and escalate concerns when necessary.
* Document patient interactions and clinical findings accurately and timely.
* Promote patient-centered care, respecting confidentiality and dignity at all times.
* Participate in ongoing professional development and maintain registration requirements.
* Support health promotion and prevention initiatives during home visits.
Person Specification
Qualifications
* HCPC Registered Paramedic; BSc in Paramedic Science (or equivalent).
* Minimum 2 years post-registration clinical experience.
* Ability to assess, triage, diagnose, and treat patients independently.
* Strong understanding of clinical guidelines and pathways.
* Excellent communication, teamwork, problem-solving, and time management skills.
* Independent Prescriber or working towards it.
* Experience in a primary care, urgent care, or PCN setting.
* Experience with EMIS or similar clinical systems.
* Knowledge of QOF, frailty management, and local services.
* Flexible, proactive, and committed to continuous improvement.
* Strong clinical judgement and problem-solving skills.
