Registered Nurse - Llantrisant Dialysis Unit

Llantrisant Dialysis Unit is a dynamic 21 station unit providing haemodialysis treatment for 84 patients in partnership with the NHS Trust. The unit operates safe staffing with a protected 3:1 patient to nurse ratio

Whether youre a newly qualified nurse, have no prior dialysis experience or returning to practice we offer a fantastic Preceptorship Programme which is designed to help you transition smoothly and excel in your career. Training is provided on a supernumerary basis by an allocated unit mentor(s), which is supported by the Practice Development Nurse team with regular on-site meetings.
Main duties of the job
* Develop and expand theoretical knowledge and practical skills involved in the care of a dialysis patient.
* Demonstrate competent and efficient patient assessment care needs, coordinating with the unit link role resources and multidisciplinary team, to ensure patients receive their optimum care.
* Demonstrate a proactive mentorship approach to the team, acting as a resource in the unit team, empowering patients, qualified nurses, ancillary staff and student nurses to share knowledge and skills.
* Promote and maintain professional relationships with both patients and Trust staff to enhance the business excellence and partnership with B Braun Avitum UK Ltd.
What you need to succeed:
* Qualified nurse with current NMC registration.
* Dedicated to providing exceptional patient care.
* Committed and eager to learn new skills.
* Ability to work independently and as part of a team.
* Aptitude for acquiring clinical skill techniques.
* Flexible to cover the needs of the patients/unit.
Working hours: 37.5 hours per week. Monday to Saturday on a rolling rota. No Sunday or night shifts!
3 x 11.5 hour shifts per week plus an additional top up shift every month
Long Day = 06:30 - 18:00
Salary band: £30,420 - £37,030. Salary will be dependent on previous dialysis experience or experience working in acute care.

Job responsibilities

To develop and expand theoreticalknowledge and practical skills involved in the care of a dialysis patient:-
- to complete the Avitum inductionprogramme within 3 days of employment start date.
- to undertake the Avitum staffdevelopment phased programme. This will be successfully completed within anagreed time frame with the Dialysis Unit Manager.
- to complete the Avitum annualmandatory training requirements.
- to attend Avitum training and studydays as scheduled.
- to achieve annual performancemanagement objectives and gateway transitions.
- optional enrolment in the access,renal & management courses.
To demonstrate an active andeffective clinical practice, providing a safe, high quality care for patients,achieving compliance with established procedures and codes of conduct andpractice at all times, recognising and accepting individual skill and knowledgelimitations and reporting when medical intervention is necessary: -
- to demonstrate commitment toself-development, updating staff development training file.
- to provide evidence of postregistration education practice as per Nursing & Midwifery Councilrequirements to maintain an active registration.
- to liaise with Dialysis UnitManager, unit trainer, mentor and Training & Development Manager toestablish appropriate annual performance management objectives.
To demonstrate competent andefficient patient assessment care needs, coordinating with the unit link roleresources and multidisciplinary team, to ensure patients receive their optimumcare :-
- to undertake a competent andaccurate clinical pre & post dialysis assessment.
- to demonstrate a competent andaccurate development of patient care plans.
- to demonstrate a competent andaccurate implementation and evaluation of patient care.
- to demonstrate a competent andaccurate monthly review of patient clinical data, providing evidence of ureakinetic monitoring and evaluation care to optimise clinical standardachievement.
- to demonstrate a competent andaccurate monthly review of the patients dialysis prescription, providingevidence of evaluation care to optimise clinical standard achievement.
- to demonstrate a competent andaccurate review of the patients coagulation management, with activated clottingtime monitoring, providing evidence of evaluation care to optimise clinicalstandard achievement.
- to demonstrate a competent andaccurate review of the patients anaemia management, providing evidence ofevaluation care to optimise clinical standard achievement.
- to demonstrate a competent andaccurate review of the patients bone disease profile, providing evidence ofevaluation care to optimise clinical standard achievement.
- to demonstrate a competent andaccurate infection control review of the patient and patients access, providingevidence of evaluation care to optimise clinical standard achievement.
- to demonstrate accurate andcompliant patient assessment documentation.
To demonstrate accurate and competentevidence of the holistic dialysis care provided as a named nurse. To maintainan in-depth knowledge of the patients physical, psychological and social state,promoting the patients interests as a nurse advocate :-
- to maintain an awareness of alldevelopments in the patients clinical timeline e.g. outpatient appointments,referrals, investigations, multidisciplinary team etc.
- to interact with the patient in theplanning of their care to optimise empowerment and independence.
- to perform documented holisticassessments of the patient at least every 3 months.
- to identify problems and potentialproblems in conjunction with the patient, providing support in developing plansto address any issues.
- to competently select theappropriate care pathway for identified problems.
- to monitor access management 3monthly and review results.
- to monitor activated clotting timeranges 3 monthly and review anticoagulation.
- to seek to interact with thepatients family in their care, as per patient consent.
- to maintain the patientsdocumentation in a compliant and legible order.
- to manage the monitoring of thepatients Hepatitis and HIV status as per policy.
- to complete, competently andaccurately, the patient manual handling assessments as per policy.
To demonstrate a proactive mentorshipapproach to the team, acting as a resource in the unit team, empoweringpatients, qualified nurses, ancillary staff and student nurses to shareknowledge and skills.
To demonstrate a proactivemotivation, competence and efficiency in the management of the unit on a dailybasis, as per development performance management objectives, to ensureconsistency, compliance and organisation are optimised on each shift:-
- to demonstrate a commitment to takecharge of a unit shift as agreed with Dialysis Unit Manager.
- to demonstrate a commitment toacquiring the knowledge & skills required to achieve this, coordinatingwith your mentor and Dialysis Unit Manager.
- to demonstrate initiative in shiftcoordination in the absence of the Senior Dialysis Nurse.
- To develop initiative and skill inthe compilation of the unit rota schedule.
To promote and maintain professionalrelationships with both patients and Trust staff to enhance the businessexcellence and partnership with B Braun Avitum UK Ltd.
To work within the Avitum Policiesand Procedures and adhere to the Quality Manual and Environmental, Health andSafety Guidelines.
