LICENSED PRACTICAL NURSE (LPN) PURPOSE: T o provide nursing and direct care to individuals residing in homes or receiving services provided by Allies, Inc. The clients we serve have varying medical needs that must be met by compassionate nurses with the skills to care for them. LPNs at Allies ensure that clients have proper health maintenance and medical treatments through their own care, and through supervision and training of the direct care staff. Requirements: Graduate of accredited Nursing School; Licensed in the State of NJ Valid Driver's License with good driving record Two years' experience in the area of Community Health and/or Home Health Care Required skills: Basic computer literacy and skills The ability to read and write English The ability to lift or transfer at least 50 lbs. The ability to sit, bend, squat, reach, and stand for several hours at a time The ability to safely and effectively execute all Personal Control and Defensive Techniques The ability to exercise good judgment and use common sense. The ability to remain calm in crisis situations. The ability to interact with a wide range of people with respect. The ability to approach daily work from a person-centered vantage point. The ability to display flexibility in work assignments and overall professionalism. Responsibilities and duties: Provide direct nursing care effectively by ensuring the general well-being of each individual served while promoting a consumer-oriented approach to health care. Administers medication and completes documentation of same. Assists individuals, as necessary in daily living skills. Accompanies individuals to and from medical appointments and activities. Accurately completes all in-house documentation. Attends conferences, seminars and training relevant to the position. Implements medically prescribed diets and exercise programs. Assists the Agency Nurse in training of Support Staff in the areas of health maintenance, medications, and medical treatments. Encourages and ensures that individuals maintain a clean, safe, and comfortable environment providing assistance as appropriate. Accurately completes all in-house documentation including but not limited to Vehicle Log, behavioral tracking, Antecedent Behavior Consequence (ABC) forms, Unusual Incident Reports (UIRs), Critical Information Log, Medication Administration Reports (MAR), Progress notes and Monthly reports as assigned. Successfully completes required residential training including: CPR and First Aid; Medication Administration; Preventing Abuse, Neglect and Exploitation; Shifting Expectations, Life Experience and Services; In House Orientation; Crisis Management; New Hire Orientation; Life Threatening Emergencies (Danielle's Law); and Corporate Compliance. Works with each individual's treatment team (IDT) and all other interested parties to build self-esteem and social capital for each individual served. Operates all types of agency vehicles, in any condition, at any point during shift.
