By focusing on proactive monitoring and support in the outpatient setting, this pathway aims to improve outcomes and empower patients through earlier detection and streamlined care. We're excited to offer a unique opportunity for a dedicated nurse to join our expanding Hepatology Nursing Service at GWH. In this dynamic role, you will: Lead nurse-ledHCC surveillance clinics for patients diagnosed with compensated cirrhosis, delivering proactive and patient-centered care. Collaborate with ourhepatology pathway coordinator to embed a new, trust-wide HCC surveillance programme, supporting patients at high risk of developing Hepatocellular Carcinoma (HCC). Participate innurse-led FibroScan clinics, helping to detect liver disease early and stratify patients for appropriate surveillance and intervention. Conductcomprehensive assessments for patients entering liver cancer surveillance pathways, planning, implementing, and evaluating tailored care plans.
