Job Title: Clinical Pharmacist - Mental Health

Role Overview
You will lead, develop, and deliver mental health clinical pharmacy services, working closely with multidisciplinary teams to provide safe, effective, and person-centred care. Opportunities are available across mental health inpatient, secure, and learning disability services.

Responsibilities
1. Implement the Medicines Management strategy within the Trust
2. Deputise for higher-grade pharmacists as required
3. Participate in rota including Medicines Information and supervision of dispensary services
4. Support the Out of Hours Medicines Advice Service and weekend working (9.30am-1.30pm) on a 1 in 12 rota
5. Conduct medicines reconciliation following patient admission or transfer, liaising with healthcare professionals
6. Ensure appropriate indication for each medicine and therapy compatibility
7. Assess the safety of existing therapies and their contribution to presenting complaints
8. Verify that new treatments are suitable for patients’ conditions
