We are seeking an experienced and motivated Support at Home Service Coordinator to join our Home and Community Care team on a full-time, maximum-term basis for 9 months, covering maternity leave. About the Role Reporting to the Support at Home Manager, the Support at Home Service Coordinator plays a fundamental role in coordinating and delivering high-quality services. This role ensures services are delivered efficiently, in line with program guidelines, quality standards and client goals. Key responsibilities include: Coordinating and scheduling Support at Home services to ensure timely, efficient and client-centred service delivery Acting as the first point of contact for clients, families, care workers, healthcare professionals and external service providers Maintaining accurate and up-to-date client records, documentation and reporting in accordance with aged care quality standards Identifying service gaps, risks and opportunities for improvement and contributing to continuous quality improvement initiatives Working collaboratively with internal teams to support positive client outcomes and a high standard of care About You To succeed in this role, you will have: Demonstrated experience in service coordination, rostering or case management within aged care or community services Sound understanding of the Home Care Standards and relevant program guidelines Strong organisational and problem-solving skills, with the ability to manage competing priorities Excellent written and verbal communication skills, with a strong focus on client engagement and service excellence The ability to work independently while contributing effectively as part of a multidisciplinary team A current Victorian driver’s licence Benefits We offer a supportive and purpose-driven work environment, including: Salary packaging benefits – up to $15,900 tax-free plus $2,650 for meals and entertainment Flexible work arrangements to support work-life balance Ongoing training and professional development, including support through aged care reforms Free on-site parking with convenient access to public transport and shopping centres If you are passionate about delivering high-quality Support at Home services and want to be part of a team shaping the future of aged care, we encourage you to apply now .
