Community Reablement Assistant x 4 (Care Worker)
Salary: Grade 3 £16,234 to £16,750 per year plus enhancements for weekend working
Location: Work will be conducted in service users’ homes across North Lincolnshire. We work in geographical teams, although the registered contractual base is Billet Lane, Scunthorpe.

The Vacancy
The North Lincolnshire Home First Community Rehabilitation service is a registered Rehabilitation and Reablement service providing time-limited interventions and therapies to people in their own homes. We focus on delivering person-centred support to help individuals remain at home confidently and safely, promoting independence.
Reablement assistant post holders will be part of our expanding transitions team, working to ensure smooth and effective transitions from hospital, with dedicated visits to support discharge from SDEC and A&E in a timely manner. They will also deliver extensive care and support within the community, helping individuals reach optimum fitness to participate in rehabilitation and supporting those requiring long-term care.
The role involves providing rehabilitation and reablement support following accidents, incidents, or illnesses to promote independence, working in conjunction with health and social care professionals.
The role also includes crisis response support for unforeseen events such as falls and carer breakdowns.
Responsibilities include supporting with moving and handling, personal care, administering medication, and promoting social inclusion and emotional wellbeing by signposting to external services.
This position offers a 25-hour weekly contract with a rolling rota, working 3 to 4 shifts per week between 07:00 and 23:00, e.g., 07:00-13:35 or 15:55-22:30. Overtime opportunities are available, especially for weekend work with enhancements.
