A Vacancy at Cambridgeshire Community Services NHS Trust.
We are expanding our Rapid Response Community Nursing Team in Luton to further support the delivery of community urgent crisis response requirements and the development of Virtual Wards in the community. This brings an exciting opportunity for experienced and skilled Registered Nurses to join the team. The Rapid Response Team in Luton is an established and successful team providing high quality nursing care to people which enables them to maximize their health status, minimize the effects of ill health and disability, and to lead as independent a life as possible. The team work collaboratively with the acute hospital, the ambulance service, GP’s and other local providers to ensure that people can receive care at home that avoids inappropriate hospital admission; the team also supports patients receiving District Nursing by providing unscheduled / urgent care in and out of hours to patients across the Luton area. Shifts covering 08:00 until 21:00.
As a senior member of the team you will lead the team by undertaking clinical assessments, planning and delivering care, demonstrating effective leadership skills and contribute to continuing service development to meet business and operational need.

You will contribute to ensuring that local policy, protocols and procedures are monitored and developed in order to provide safe, effective care including the provision of assessment and prioritization of patient referrals, plan and deliver treatment plans that ensure effective, timely urgent care in and out of hours.
You will have passion and drive with a strong team ethic and flexible disposition, responding positively to a fast-changing environment.
You will need to have strong clinical leadership skills and be able to work to the challenges that a dynamic work environment brings.
You will have experience of autonomous working, have a sound up to date clinical base knowledge and be confident and competent in clinical assessment and decision making.

As a senior member of the team you will lead the team in providing clinical assessments demonstrate effective leadership skills and have the ability to support the Team Leads in the continuing service development and delivery of our ambition for teams to work more closely together.
