In the East Hereford Neighbourhood Mental Health Team (NMHT) we strive to improve the mental health and wellbeing of our local communities. Responsibilities
Main duties of the job include being a key member and resource of the MDT core team. Aspects of the role also include close working within the MDT, undertaking and supporting individuals accessing the service in Recovery focussed activities. There is scope for professional development as the trust has a repertoire of training available to those wishing to extend their skill set. Developing our workforce is important to us, and we are pioneering champion roles across the organisation. Support registered professionals to deliver evidenced based interventions, managing a caseload of patients; supporting and signposting to improve social isolation and improved population health. You will have experience of working with this user group in community settings. The ability to work flexibly as the need of the service demands is essential. Attend weekly MDT meetings, participate in professionals meetings, signpost to adjunct services depending on patient need. Provide support and guidance to the carers of patients accessing the services.
