Job Summary
We are looking for a Band 3 Support Worker to join our nurse and therapist led Intermediate Care Unit. This role focuses on rehabilitation and reablement with a specialist palliative care component. The successful candidate will work as part of a multidisciplinary team to provide person‑centred care to patients progressing through recovery or at the end of life.
The position is a full‑time or part‑time permanent role (Band 3) and requires availability for nights, weekends and bank holidays, as it is a 24/7 service. No visa sponsorship will be offered; candidates must have the right to work in the UK without employer sponsorship.
Key Responsibilities
* Lead delegated proactive interventions, including reablement and therapy exercises, to support patients with self‑care and independence.
* Provide compassionate support and care to patients in the final stages of life.
* Assist therapy staff with home visits and support the delivery of physiotherapy, occupational therapy and nursing programmes.
* Ensure safe, timely, and accurate clinical care within your competency, including wound care, catheter care, venepuncture, and skin assessments.
* Record and report clinical observations using NEWS2, blood pressure, pulse, oxygen saturation, glucose, and other relevant parameters.
* Support and deliver aspects of activities of daily living, including nutrition, continence management and toileting programmes.
* Engage patients, relatives and carers in effective communication, respecting confidentiality and cultural diversity.
* Participate in one‑to‑one meetings, clinical supervision and ongoing learning opportunities.
* Contribute to clinical governance via audit participation and the maintenance of accurate, legible patient records in line with ECCH policy.
* Report changes in patient condition to registered health care professionals.
* Use clinical equipment safely and contribute to efficient resource management.
Core Clinical Skills
* Pressure ulcer prevention and management
* Wound care, skin tear pathway and simple wound cover change
* Catheter care and bladder scanning
* Venepuncture and specimen collection
* Portable ultrasound (basic urinary) and urinalysis
* Documentation of NEWS2, blood glucose and other vital signs
* Support for physiotherapy and occupational therapy programmes
Qualifications & Knowledge
* Essential Qualifications
* Level 2/3 NVQ Health related or equivalent experience in Health or Social Care
* Care Certificate (mandatory upon employment)
* GCSE level 5 in English and Mathematics or equivalent
* Good written and verbal communication skills. Good interpersonal skills and ability to listen.
* Basic email and information technology skills.
* Common sense and practical problem‑solving approach.

Working Pattern & Contract
Contract: Permanent. Working pattern: Full‑time or Part‑time, flexible to cover 24/7 service requirements.
Salary: £24,937 to £26,598 per annum (Band 3).
