SUPPORT PLANNER LHRSP LINCOLN DISPERSED, LINCOLN, PERMANENT, 37HRS, GRADE 3, £25,235, FTE We are currently recruiting for a Full Time Support Planner position. LHRSP Lincoln Dispersed is a community-based support service for people with complex needs using our 40 units of Dispersed Accommodation across Lincoln City working Monday-Friday. We accommodate individuals for 6 months and work with them to get appropriate permanent accommodation. We work in partnership with various agencies providing support to address key problems with housing, benefits, debt, offending, substance misuse and help them develop knowledge, skills, and resilience to remain safe, well and independently in accommodation. In this role you will use your experience of assessing service user needs and planning individually tailored support for people to help them achieve their goals and help them review their progress and achievements. You will be expected to work effectively within a team, carry your own caseload and support your colleagues. You will be expected to have a good working knowledge of the needs of people struggling with their mental health and substance misuse, housing law, debt support and issues related to the persons ability to live independently.
