Healthcare Support Worker - Hospital at Home – NHS Scotland
The Hospital at Home service provides secondary level care within the patient’s own home or care setting. The Health Care Support Worker (HCSW) will work as part of the Hospital at Home team, providing clinical care to patients who would otherwise be in a hospital setting. This post is integral to ensuring safe, effective, high quality care delivery in the patient’s home environment.
Full and part time hours available
Responsibilities
* As an HCSW you will liaise with several professionals/agencies, both internal and external daily including GP practices, local authority teams and the voluntary sector.
* As part of a multidisciplinary team, the post holder will carry out personal care duties for patients, in support of the registered nurse and other relevant professional practitioners, where appropriate.
* As an HCSW you will be responsible for looking after a group of frail, elderly patients under the supervision of a registered nurse. They will be for delivering care in a home environment.
* Currently the service is 7 days, 0800-1800 daily. As a HCSW within H@H your post would work Monday – Friday with weekend working.
