Are you a dynamic, compassionate, and highly motivated nurse with advanced knowledge and skills in palliative care? If so, an exciting opportunity has arisen for a part-time Band 6 Clinical Nurse Specialist to join the busy and dedicated Community Palliative Care Team (SPCT) in Pembrokeshire.

This is a rewarding opportunity to be part of a team committed to improving the quality of life for individuals—and those closest to them—who are living with progressive, life-limiting illnesses. The role focuses on delivering expert symptom management, psychological support, and holistic care, with a strong emphasis on palliative and end-of-life care within the acute setting.

Main duties of the job

Key Responsibilities:


* Deliver high-quality, compassionate care and specialist advice to patients, families, and caregivers.
* Communicate sensitively and effectively with patients, families, and multidisciplinary teams.
* Support and educate colleagues to ensure holistic care planning, including advance care planning and DNACPR discussions.
* Identify patients with palliative care needs early to prevent unnecessary delays in treatment
* Manage a defined caseload under the guidance of the Consultant, working closely with Clinical Nurse Specialists, doctors, therapists, and the wider community MDT.
* Contribute to service improvement, education, audit, and research initiatives.

What We’re Looking For:

* A registered nurse with significant experience in palliative care.
* Excellent communication skills and emotional intelligence.
* A passion for delivering proactive, person-centred care.
* Ability to work both independently and as part of a multidisciplinary team across the community.

The ability to speak Welsh is desirable for this post; however, both English and Welsh speakers are encouraged to apply.

Working for our organisation

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:

Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;

Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;

48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;

Numerous locations providing mental health and learning disabilities services;

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
