Clinical Ward-Based Services
* Delivering a high-quality, patient-focused clinical pharmacy service on wards, ensuring the safe and effective use of medicines.
* Supporting Timely and Safe Discharges:
o Facilitating early discharge and reducing hospital length of stay by optimising patients drug therapy.
o Providing tailored education on medicine use, including support with self-medication and assessing the need for adherence aids.
o Offering comprehensive counselling to patients about their medications prior to leave and discharge.
o Ensuring continuity of care through robust discharge planning and communication with community healthcare providers.
* General Pharmacy Duties:
o Participating in wider clinical pharmacy and departmental responsibilities as required, supporting the overall objectives of the Trusts pharmacy service.
