About the Job
Adult Social Care Practitioner in the Neighbourhoods East team works closely with health and community colleagues to support people and families so they can remain living in their own community networks, using a strengths‑based approach.

* Salary: £28,598 rising to £32,061 annually
* Full time – 37 hours per week
* Hybrid working opportunities
* Locations: Yeovil, Mendip
* Vehicle access required
* 28 days annual leave (pro rata for part time)
What will I be doing?
* Work closely with health and community colleagues to support people and families to maintain independence.
* Encourage independent living, build confidence and use a strengths‑based approach.
* Identify concerns, needs and existing support, and help develop solutions.
* Build knowledge of local area and work in partnership with local agencies.
* Contribute to developing the new Adult Social Care service.
* Provide creative solutions for individuals and families.
Qualifications & Experience
* Desirable to have or be working towards an NVQ Level 3 in Health and Social Care or equivalent (health and social care related).
* Experience in a caring or supportive role.
* Willingness to innovate and challenge the status quo to improve support.
* Passionate about advocating for others.
* Solution‑focused, creative and independent.
