Main duties of the job
Provideassessment, diagnosis and treatment at first point of contact by attending topatients according to their needs in the practice. Act as an autonomousclinician, with the support and guidance from the GPs
Proactivelyidentify, diagnose and manage treatment plans for patients at risk ofdeveloping a long-term condition (as appropriate).
Diagnoseand manage both acute and chronic conditions, integrating both drug andnon-drug based treatment methods into a management plan.
Accuratelytriage and prioritise patients, including in emergency situations,demonstrating the use of a variety of techniques to elicit the history of anevent/illness, including past medical and drug history.
Recognise,assess and refer patients presenting with mental health needs
Telephonetriage as appropriate for clinics and home visits and provide access to GPs asnecessary.
Evaluateclinical information from examination and history taking and initiateappropriate treatment and / or referral, including admission to hospital ifrequired.

Job Summary
To provide a high-quality,cost-effective service to the patients of Howard House which is part of SuffolkPrimary Care diagnosing and prescribing treatments and medication while workingwithin defined procedures and protocols as laid down by the GPs.
To work as part of a clinicalteam, with GPs, Advanced Nurse Practitioners, Physician Associates, PracticeNurses and Healthcare Assistants.
To be responsible for seeing patientsin the practice or by telephone, who present with a wide range of minor illness,urgent and same day requests for treatment and chronic disease management;providing care for the presenting patient from initial history taking, clinicalassessment, diagnosis, treatment and evaluation of care.
The post holder will need todemonstrate safe, clinical decision-making and expert care, includingassessment and diagnostic skills. There will be the need to prioritise andtriage the needs of patients accordingly making any necessary referrals forinvestigations in the appropriate manner.

Signpostappropriately to other services in the identified healthcare pathway and torefer patients to appropriate care settings, within locally agreed healthcarepathways.
Maintainaccurate and systematic documentation of patient interactions and clinical careprovided, including adverse events.
Advise andsupport members of the public on healthy lifestyles, health promotion anddisease prevention.
Provideclinical advice to other members of the team where appropriate.
Supportthe GPs in other duties as required for example assisting with ECGs or othernursing duties where a nurse is not available.

To plan and organise own workload, and to supportcolleagues as well as patients and carers.
To attend meetings of relevance as agreed with thepost holders line manager.

Workcollaboratively with other members of the health care team such as the, staffin Care Homes, Ambulance Service, and Secondary Care etc.
Ensurethat the wellbeing of patients and staff is maintained in accordance withPractice policies. To undertake clinical audits in line with the organisationsclinical audit plan, and to participate as required in equipment testing andother research.
Contributeto the analysis of information to inform future development of the service.
Participateand contribute to clinical meetings
Identifyrisks to service provision and highlight them to the Clinical Lead.
Workwithin the requirements of Data Protection legislation as it applies to patientrecords.

6. RCN Accredited Nurse Practitioner Module
7. Registration as an independent non-medical prescriber
