Overview
The Skin Cancer Support Worker will be based within the Skin Cancer Nursing Team's establishment. This role provides support to patients and the team of specialist nurses caring for patients with a skin cancer diagnosis. The post is predominantly patient-facing, entailing support to patients via email or telephone and signposting to appropriate services for patients and their families/carers. It also entails organising and assisting the clinical nursing teams to deliver the Cancer Nursing service effectively and efficiently, ensuring patients' queries and care are actioned in a timely manner. The post will be part of the growing group of cancer healthcare support workers leading on and supporting personalised care to help patients and their families live with their cancer diagnosis.
* To provide general, specific and specialist information to patients and clinical teams regarding skin cancer and cancer services, to ensure patients are signposted during treatment and beyond.
* Provide information and support for people with non-complex care needs.
* Engage with community services as appropriate.
* Record patient information on the Trust's systems.
* Assist with admin tasks related to the Skin Cancer tracker to ensure smooth running of this service, documenting on the appropriate IT systems.
* Contribute to maintaining and organising the day-to-day running of the Skin Cancer service, including handling phone calls, referrals, emails, and patient queries.
* Carry out holistic needs assessments (at times using e-HNA on an iPad) either by telephone or face-to-face and signpost according to patients' concerns.
