About the Role
We are seeking an experienced and motivated Social Worker to join Medway's Hospital Integrated & Community Discharge Team, supporting the delivery of high-quality adult social care across both acute and community settings.
This is an exciting opportunity to be part of a forward-thinking service at the forefront of the Discharge to Assess model. You'll work with individuals and families during hospital admissions and in the community following discharge - whether in a health bed, social care step-down placement, or their own home. You will also play a key role in the emergency department alongside our health colleagues, ensuring timely, safe, and person-centred discharges.
Responsibilities
* Support timely and safe hospital discharges through integrated working.
* Carry out assessments and interventions in acute hospitals, community settings, and service users' homes.
* Apply strength-based, personalised approaches to improve outcomes.
* Work within the framework of the Care Act, Mental Capacity Act, and Mental Health Act.
* Collaborate with colleagues and partners to deliver effective, resource-conscious care.
* Promote and uphold safeguarding policies and the welfare of vulnerable adults at all times.
About You
* Experience in adult social care, ideally in hospital or discharge settings.
* Strong knowledge of relevant legislation.
* The ability to work in a fast-paced, dynamic environment while maintaining professional standards.
* A commitment to person-centred practice and integrated partnership working.
