All About The Role Are you an experienced leader ready to make a difference in adult social care? We are looking for a Team Manager to lead our Continued Healthcare (CHC) services and Discharge to Assess pathway. This is a pivotal role focused on effective CHC coordination across our system. You will manage an Adult Social Care team supporting people discharged from hospital on a D2A Pathway and those referred to Adult Social Care for participation in the Continuing Health Care process. Working with the Head of Service, you will drive performance and improve outcomes for residents. You will also ensure strong collaboration with health partners and robust CHC processes.
