An exciting opportunity has arisen for a Band 6 Mental Health Practitioner (Mental Health Nurse, RGN, Social Worker, OT or allied health professional) to join our High-Volume User service.
The High-Volume User (HVU) team currently provide assessment and community support to patients who frequently attend the Accident and Emergency Department at the University Hospital North Midlands (UHNM). High intensity users of A&E departments have been identified as having a high impact on the wait times and flow of the hospital and other emergency services and health systems. If this patient group are not linked into services, A&E can become the default position. Vulnerable patients who attend A&E due to a combination of physical, psychological, and social factors often require a multidisciplinary team approach to tackle their complex and diverse issues. The HVU team will support high intensity users of A&E, linking them into the most appropriate community service in the health, mental health and/or social care or community system that is best able to meet their needs.
Main duties of the job
With a variety of management techniques and interventions, the team offers individuals with complex mental and physical health difficulties clinical and practical support. Candidates must have a flexible approach that adapts to the needs of patients, their families, and carers. Working as a mental health professional in a reputable team, the role entails assisting a complex caseload of clients with a range of mental, physical, and social needs in addition to serving as the clients allocated practitioner.
In order to help people and enhance their overall quality of life, practitioners will work closely with them in their homes or communities. This position includes patient assessment, care/support, risk assessment development, collaborative care planning, and patient document management. In addition, you should become an advocate for the individuals in your care, offering them support and guidance.
We are looking for a motivated individual with strong teamwork abilities who will foster teamwork, respect, honesty, accountability, excellence, and compassion. We're seeking someone who wants to expand their knowledge and skills in this field. You should be a competent and motivated practitioner with relevant proven experience who aligns with the Trust's principles and your governing Professional body. You will be part of a multidisciplinary team that provides community assessments and therapeutic interventions.
