Job Summary:

To provide outreach, engagement, and advocacy for homeless individuals and families who are referred into PCHO’s housing programs and/or are currently in housing programs provided by PCHO. Coordinator will follow the Housing First Model when working with individuals who enter PCHO programs. Tasks include outreach, assessment intake, case management, skill building and other supportive services to assist persons eligible for housing services in the Rochester and Monroe County Community. Assist program participants in securing and maintaining permanent and affordable housing through PCHO housing programs and establishing and maintaining benefits such as DHS, SSI, SNAP, Employment. Assist program participants in meeting their personal life goals in a manner that is consistent with the culture, values and beliefs of the participants.

Qualifications:

* Bachelor’s degree in Social Work or Human Services related degree preferred OR Associate’s degree in Human Services or related area with 2 years full-time work experience or its part-time equivalent working with persons that were homeless or underserved OR High School Diploma with at least 4 years full-time work experience or its part-time equivalent working with persons that were homeless or underserved will be considered
* Possess social justice and human rights perspective and frameworks that align with agency's principles and mission
* Knowledge of community resources and social work practices with homeless or underserved populations
* Ability to work independently with strong focus, task-oriented, non-judgmental, and able to set clear boundaries
* Possess a strong sense of respect for confidentiality involving both participants and peers
* Possess an ability to work in a variety of settings with culturally diverse individuals and families, and remain sensitive to their individuality
* Knowledge of and/or informed practice of motivational interviewing, trauma-informed care, etc.
* Possession of a valid NYS driver’s license and a driver’s record considered acceptable by agency and insurance carrier and availability of a car for daily use including transporting clients to appointments
* Effective oral and written communication, ability to establish priorities, work independently, manage multiple tasks
* Excellent engagement, written and verbal communication skills and documentation skills

Essential Responsibilities and Functions:

* Provides ongoing case management and other supportive services to unsheltered households entering the program and formerly homeless households currently residing in the program
* Provide detailed and timely documentation into the HMIS, ensuring each client is entered thoroughly and within 72 hours of contact which includes Individualized Service Plans and monthly contact notes
* Coordinates new participant intake appointment with Referring Case Manager (RCM) and PCHO’s Housing Specialist (HS), working closely with the HS during the intake process fostering a positive relation with each participant. During the intake process, along with the HS, assist to ensure participants understand the occupancy agreement. Assists in housing search during intake process and up to and including exiting from the program
* Works closely with RCM to ensure all required documentation is gathered and takes the lead with housing search, facilitating participant’s acceptance of unit prior to HQS inspection. Once participant commits to unit, coordinate move-in
* Holds monthly in-person meetings with assigned participants to discuss progress and examine condition of unit. Reviews Individualized Service Plans with participants quarterly
* Maintain positive relationships with landlords, triage or troubleshoot any tenant issues providing direct advocacy as needed, as well as educating participants on how to advocate on their own behalf
* Provide intervention services according to identified needs as necessary for delivery of service, as well as assisting with transportation and access to ongoing scheduled appointments as needed
* Assist participants with nutritional and personal hygiene needs/education and other daily living skills. Advocate for participants with other agencies and landlords as necessary
* Assess each participant for needed other supportive services. Other supportive services include any services that would assist participants in successfully remaining housed in their unit (i.e., health home care management, peer support, mental health/SUD)
* Assist participants in accessing benefits including DHS, SSA, SNAP and any other necessary resources needed to achieve sustainable housing. Complete necessary steps to ensure household pays portion of rent (i.e., establishing budget, accepting rent payments when applicable)
* Other duties as assigned by supervisor
