Social Worker in Mental Health Discharge Team, East Sussex, earning £32 per hour. Fixed term contract for 12 months. What you will do:
* Facilitate effective and timely discharges from acute hospitals for individuals needing mental health support
* Collaborate with Sussex Partnership Foundation Trust and voluntary sector partners to create personalised discharge plans
* Build and maintain positive relationships with clients, their families, and other health and social care professionals
* Conduct assessments and reviews to ensure the safe transition of individuals back into the community
* Participate in multidisciplinary team meetings to contribute to the planning and implementation of discharge strategies
Experience we are looking for:
* Registered Social Worker with Social Work England; please include your registration number in your application
* Completion of the Assessed and Supported Year in Employment (ASYE)
* Self‑motivation and commitment to personal development and training
* Ability to travel as required by the role
* Strong communication skills and the ability to work collaboratively
