Adults Social Worker - Early Help & Wellbeing, Chelmsford
Role open to Newly Qualified Social Workers (NQSW). The starting salary for an NQSW is £34,902 per annum, rising to £37,185 per annum upon successful completion of the ASYE programme.
About the Team
The Early Help & Wellbeing Team is a welcoming, compassionate team offering preventative and early help to connect people to opportunities in their communities and provide support to achieve personal ambitions, goals and aspirations. Mental health social care is transforming into a new operational delivery model focusing on early intervention, prevention, information, advice and guidance, and improving wellbeing outcomes. The service includes direct work with adults and care act assessments and reviews to support wellbeing outcomes.
Principles include Prevention, Early Intervention, Enablement and Safeguarding. The role supports people, families and carers to access information and tools to live healthily and independently.
You will balance direct work to support people with assessments/reviews for eligibility for commissioned services, while balancing need with finance. Flexible hybrid working patterns (Mon-Fri) are essential to meet service demands.
Responsibilities
* Develop strong working relationships with partners, other agencies and the voluntary sector to deliver improved outcomes.
* Assess complex social care needs and provide support to vulnerable adults to enable healthy, independent living.
* Develop client-centred support plans with commitments to solutions that meet individual needs.
* Work within statutory frameworks, policy and guidance and determine thresholds for statutory intervention.
* Conduct assessments and plan responses to safeguard children or vulnerable adults when required.
* Promote early intervention and prevention, increasing independence and working with families and communities.
* Understand and apply the Care Act (2014) and related legislation; work collaboratively with health and other professionals.
* Provide direct work to support mental wellbeing and knowledge of neurodivergent conditions; offer advice and guidance.
* Review care and support in line with guidance and ensure appropriate documentation and records are maintained.
* Support other health partners with understanding of statutory responsibilities for carers’ needs.
* Demonstrate ongoing professional development and adherence to professional standards.
* Be mobile across the Essex County Council area as required; driving licence and access to a vehicle or suitable alternative mobility solution is essential.
Qualifications & Experience
* Qualified social worker with current Social Work England registration.
* Experience in working within mental health and delivering direct work, with evidence of continuing professional development.
* Understanding of current legislation, regulations and guidance; ability to chair meetings involving clients, families, carers and multi-agency professionals.
