Given the seniority of this role, candidates with 5+ years’ experience post-CCT are preferred, unless there is significant prior experience in geriatrics/frailty care.
You must be qualified as a General Practitioner and registered with the GMC and UK performers list.
Salary – c£130,000-150,000 per annum
Location – Bodmin / Fowey / Lostwithiel areas

* Working alongside care coordinators, nursing teams and acute visiting clinicians
* Reporting into the PCN Strategic Management team including Clinical Directors

The responsibilities –
* Leadership & Service Development: Lead the development and ongoing improvement of the Integrated Neighbourhood Team (INT) Frailty Team, focusing on admission avoidance, effective discharge planning, and coordinated care across system partners.
* Clinical Oversight & Coordination: Provide clinical leadership for daily MDT huddles, support ‘hospital pull’ processes, and ensure safe, timely discharges with appropriate follow-up to reduce re-admissions.
* Patient Care & Case Management: Deliver high-quality medical care including diagnosis, prescribing, care planning for at-risk patients, and long-term condition management, using Ardens Templates and accurate clinical coding.
* Multidisciplinary Collaboration: Work closely with geriatricians, primary care teams, and community services to deliver integrated care planning, prevent unnecessary hospital admissions, and promote shared decision-making.
* Governance, Improvement & Training: Contribute to clinical governance, audit, and continuous quality improvement; support the training and development of clinical staff and students within the team.
* Operational & Strategic Contribution: Manage clinical administration, triage, and resource prioritisation effectively; support the wider strategic objectives of the INT and ICS through collaboration, reporting, and adherence to protocols.
The set-up
* Ideally full-time (8 sessions) but will consider a job share
* Roughly 50/50 in terms of desk-based and travel to other sites/meetings
* 12-month initial contract with excellent scope for a further 12 months and likely a permanent role beyond that
* High degree of flexibility over working hours
