An excellent opportunity has arisen for a motivated and enthusiastic Band 3 Health Care Support Worker to work in the Locala Community Heart Failure Team in Huddersfield.
The service is delivered Monday to Friday, 8am to 5pm and this post is a part time with hours negotiable – up to 30 hours.
The post holder will be required to work with the specialist nurses, supporting excellent holistic care to patients with heart failure. This will involve working in a wide variety of clinic and community settings, taking blood, observations and other clinical tasks to support patient optimisation. The post holder will also support the nurses in delivering their care, contacting patients, arranging clinics and home visits, as well as organising their own list of visits and delegated tasks.
We are looking for an individual with established skills in completing phlebotomy and patient observations who is confident working as part of a team and alone in a variety of settings.
