MSK Trailblazer Team Leader
Salary: £40,505 – £44,350 per annum
Location: West & North London (community hubs, with regular travel required)

We’re looking for a Trailblazer Team Leader to join the growing Community Health & Wellbeing division and lead a team of Health & Employment Coaches delivering this innovative government-funded programme.
About the Trailblazer Programme
Trailblazer integrates musculoskeletal (MSK) health recovery and employment support into one holistic pathway. We partner with MSK teams, GP practices, and other referral agencies to help people who are unemployed – or struggling in work due to health conditions – to return to, remain in, or find new employment while improving their overall wellbeing.
About the role
As Team Leader, you will:
• Manage, support, and develop a team of Health & Employment Coaches, providing supervision, caseload reviews, mentoring, and performance management.
• Work closely with clinicians and multi-disciplinary teams (physiotherapists, nutritionists, occupational health specialists, etc.) to ensure participants receive seamless, joined-up support.
• Lead your team to meet performance targets, ensuring at least 35% conversion of programme starts into job outcomes.
• Build strong relationships with NHS partners, referral agencies, and employers to promote the service and secure sustainable opportunities for participants.
• Deliver high standards of safeguarding, compliance, and data accuracy across the programme.
This is a hands-on leadership role requiring visibility across community hubs, regular travel across West and North London, and active engagement with participants, partners, and employers.
About you
We’re looking for someone with
• Proven experience in team leadership within a high-performance environment.
• Experience of managing staff who support participants with health and/or employment barriers.
• Strong knowledge of how common health conditions – especially MSK – impact work, and the ability to support tailored return-to-work plans.
• Excellent organisational, coaching, and performance management skills.
• Confidence in building partnerships with clinicians, community services, and employers.
• Strong IT, communication, and customer service skills.
You will also need
• A willingness to complete NHS-accredited Health Coach training (if not already qualified).
• Enhanced DBS clearance (required).
• Flexibility to travel regularly across West & North London.
