Overview
As an Advanced Nurse Practitioner, you will deliver high‑quality, patient‑centred care while working autonomously within your professional scope. You will assess, diagnose, and manage patients, making clinical decisions in line with established procedures and protocols set out by our GPs.
Key Responsibilities
- Diagnose and treat patients independently
- Prescribe medications safely and appropriately
- Request and interpret investigations
- Deliver evidence‑based interventions to optimise patient outcomes
- Work collaboratively with the multidisciplinary team to ensure excellent continuity of care
Clinical Responsibilities
- Receive patients with undifferentiated and undiagnosed problems and assess their healthcare needs.
- Provide urgent, same‑day, and routine appointments for patients of all ages.
- Triage patient queries and manage clinical workload efficiently.
- Provide health promotion, disease prevention, and lifestyle advice.
- Assess, manage, and treat minor injuries and illnesses.
- Safeguard children and vulnerable adults, following local and national policies.
- Prescribe medications and treatments independently within professional competencies.
- Assess, diagnose, treat, prescribe, refer and discharge as appropriate and deliver key elements of National Service Frameworks, providing public health, health protection and promotion programmes that improve health and reduce inequalities.
Administrative Responsibilities
- Maintain accurate, legible, and contemporaneous patient records.
- Support delivery of QOF and other contractual requirements.
- Collect data and participate in regular practice audit.
- Contribute to the development of practice policies and service improvements.
Other Responsibilities
- Work closely with GPs, nurses, pharmacists, social prescribers, and external services to ensure integrated patient care.
- Liaise with secondary care, community teams, and allied health professionals as needed.
- Participate in MDT meetings, case discussions, and safeguarding forums.
- Support the development of protocols, pathways, and best practice guidelines.
- Maintain a safe and therapeutic environment for all Primary Health Care Team staff, patients, carers and co‑workers from other agencies.
- Ensure that all statutory regulations such as Health & Safety and COSHH are implemented and adhered to.
- Promote a professional approach at all times by attire and attitude to patients and staff both in and out of the practice.
- Maintain own continuous professional development and ensure PDP is kept up-to-date in line with the requirements of the practice.
- Attend and participate in staff meetings.
