About Us:
Complex Case Management is a leading provider of tailored, person-centred care in the community across the Northwest. We are currently seeking an experienced support worker who is sociable, enthusiastic and motivated to assist a 19-year-old male within his home and the community to fulfil his goal studying at university. He sustained a brain injury at birth so although he is mobile, verbal and intelligent, tasks and activities which are out of his comfort zone can be overwhelming, so he will need a friendly, knowledgeable assistant to step in and support to lighten the load. He has some difficulties with epilepsy, organisation, planning, working memory and social engagement, but he likes to work hard on his studies and go out and about. He needs a support worker with excellent organisation, communication and interpersonal skills.
He enjoys studying, stimulating conversations, interested in languages and travel, going to the gym, visiting the theatre, watching films, YouTube videos and cooking. He is keen to resume his exercise regime at the gym to aid his mobility and fitness but will require encouragement to embed this activity into his weekly routine.
Full training will be provided to any successful candidate to enable them to support him to achieve his rehabilitation goals and any clinical needs.
The successful candidate will have the following skills and attributes:
· A good temperament and patience, to be able to manage the client's sensitive / neuro diverse needs.
· The enthusiasm and energy to motivate and support the client to with his therapy program including physiotherapy, occupational therapy, speech and language therapy and neuropsychology.
· Suitable skills to be able to identify risks and dangers to the client in relation to his epilepsy presentation.
· An attentive attitude and the ability to listen and understand the client's sensitive needs, with good patience and reassurance skills necessary to provide the correct response.
The post requires that you:
· are a non-smoker
· work independently to a great extent
· can cover weekdays
· can work flexibly, along with the ability to work additional hours as required
The post is part time, permanent work for the right candidate.
1 position available: average of 20 hours per week Monday – Friday (5 hours per day, 4 days per week) exact days to be confirmed.
Rate of pay: £15 per hour

Key Tasks and Responsibilities:
Specific:
· Maintaining the young person's goals to attend university and assist with organisation of studies
· Maintaining the young person's goals with mobility and fitness
· Maintaining the young person's general health
· Maintaining the young person's nutrition which may involve batch cooking
· Supporting the young person's therapeutic engagement
· Supporting engagement in appropriate social and leisure activities for the young person
· Maintaining the young person's safety in all environments
General:
· Promote, monitor and maintain health, safety and security in the workplace.
· Promote effective communication and relationships.
· Contribute to the protection of individuals from abuse.
· Establish and maintain a trusting, therapeutic, working relationship
· Be responsible for the young person's personal welfare
· Travel to hospitals or elsewhere for therapeutic care
· Liaise with other agencies as required
· Complete daily records as advised including time sheets, daily record sheets, petty cash sheets and critical incidents.
· Attend Team Meetings/review meetings with the CCM and other persons as nominated by the employer
· Attend for regular Formal Supervisions and Annual Appraisal
· Attend training sessions as arranged
