A rare and exciting opportunity for a Community Nurse has arisen within our West Suffolk Community team for adults with learning disabilities and autism. We are seeking enthusiastic and highly motivated nurses to join our dynamic multi-disciplinary service. You’ll be joining an innovative team of nurses, working collaboratively with multiple agencies and professional clinicians. Our service covers the geographical area of West Suffolk and you’ll be responsible for managing a caseload from our base in Bury St Edmunds. The development is supported by investment and new service structures aligned with the Transforming Care Agenda and a fresh approach to learning disability and autism care.
Responsibilities
Main duties of the job include thinking and working creatively with individuals and their carers, carrying out comprehensive assessments and planning, implementing and evaluating delivery of nursing interventions as part of a holistic approach. You will be able to apply your nursing skills to a wide range of situations, working with adults with learning disabilities living in the community. You will work as part of a team and also be able to work independently.

* Enjoy working as part of a team and with colleagues from a range of disciplines
* Ability to work independently
* Experience in assessing, planning, implementing and evaluating nursing interventions
* Experience with adults with learning disabilities living in the community is desirable
* Commitment to ongoing learning and professional development
