37 hours per week We are looking for a skilled and compassionate professional to join our team, where you will play a vital role in supporting adults with a range of complex and challenging needs to live well within their communities. In this role, you will manage a varied caseload and undertake high-quality, person-centred assessments, independence planning and reviews in line with legislation including the Care Act and Mental Capacity Act. You will work collaboratively with individuals, carers, families and partner agencies to develop outcome-focused plans, manage risk, respond to safeguarding concerns and provide timely crisis intervention when people need support most. You will also ensure care packages are effective, proportionate and make best use of available community, third-sector and commissioned services.
