We are seeking a dedicated Social Worker to join our Permanence team, focusing on ensuring the long-term stability and well-being of children who cannot be looked after by their parents. This role requires a professional who is adept at making robust decisions about the future of children in care, considering various permanence options, and providing support and stability through well-developed care plans.
Day to Day of the role:Review case notes, liaise for additional information, and evaluate support for newly qualified social workers.Undertake assessments of support needs and develop Care & Support Plans for Service Users, Carers, and Families.Produce, deliver, evaluate, and update Care & Support Plans to resolve social care, health, and well-being difficulties.Work with Service Users, Carers, and Families to make informed choices and decisions.Proactively handle Safeguarding issues and follow relevant policies and procedures.Identify, assess, and manage risks, and respond to crisis situations with appropriate legal intervention.Maintain high data quality on social care systems and ensure confidentiality of personal information.Prepare for and participate in meetings and decision-making forums, advocating on behalf of Service Users, Carers, and...
