<div><p><b>Join Our Team as a Band 5 Community Staff Nurse - West Locality</b></p><p>We’re looking for dynamic, forward-thinking Band 5 Community Staff Nurses to join our West locality Community Nursing Team within the Integrated Nursing and Conditions Services at <b>CHCP CIC</b>. We currently have a <b>full-time position</b> available.</p><p>As part of our integrated community nursing team, you’ll deliver high‑quality, holistic care to people in their own homes, helping them stay well, safe, and supported in their community.</p><h3>Main duties of the job</h3><ul><li>A proactive, supportive, and innovative nursing team</li><li>Opportunities to work collaboratively with health and social care partners</li><li>A role that encourages autonomy, initiative, and new ways of working</li><li>Full training, mentorship, and ongoing professional development</li></ul><h3>About you:</h3><ul><li>Passionate about providing excellent community‑based nursing care</li><li>Ready to work flexibly and independently as part of a multidisciplinary team</li><li>Community nursing experience is helpful, but not essential— we’ll support your development</li><li>Able to travel across the locality with access to suitable transport</li></ul><h3>Qualifications</h3><ul><li>Degree in Nursing Studies / First level Registered Nurse or working towards final assessment to achieve first level registration</li></ul><h3>Knowledge</h3><ul><li>Demonstrate an understanding of the role of a community nurse</li><li>Knowledge of legislation impacting on service delivery</li><li>Knowledge of the modernisation agenda</li><li>Demonstrate an understanding of evidence based practice</li><li>Knowledge of local initiatives</li><li>Demonstrate an understanding of the MDT</li><li>Basic IT skills</li><li>Knowledge and skills in the assessment process</li><li>Able to mentor students</li><li>Evidence of professional development</li><li>Have a working knowledge and understanding of First Visit Criteria</li></ul><h3>Experience</h3><ul><li>Basic health and safety awareness</li><li>Experience of working within a healthcare setting</li><li>Experience of working with families and carers</li><li>Experience of working in a team</li><li>Experience of working with computerised recording systems</li><li>Experience of multi‑agency working</li><li>Experience of working within a community setting</li><li>To have recently worked within a nursing capacity</li><li>To be able to provide education to patients, carers, staff and other health care professionals</li></ul></div>
