Job Summary
We are looking for a highly motivated and enthusiastic Assistant Practitioner to work within the Rehabilitation Team.
The candidate must demonstrate the ability to treat patients effectively with excellent communication skills and the ability to work well within the wider multidisciplinary team. They will provide planned care to patients undergoing cancer treatment, symptom control, palliative care and rehabilitation and be able to take appropriate action to achieve the planned aims. They will work alongside other staff members and disciplines to ensure the Rehabilitation Unit remains a safe patient environment. The Assistant Practitioner will be proficient, competent and require minimum supervision.
Main duties / Responsibilities
* Maintain regular and close communication with physiotherapists and team leader.
* Work alongside the MDT to deliver programmes of care and therapy for treatment, as per plan of care determined by physiotherapists.
* Develop a rapport with patients and encourage interest and participation in their treatment programme.
* Support and undertake the assessment and management of non-complex patients’ rehabilitation needs under the direct/indirect supervision of the physiotherapist, including data collection, mobility/basic function assessment, implementation of treatment plans, monitoring progress, feedback with seniors, written reports and completion of MDT referrals.
* Support and assist physiotherapists within medical and surgical teams in the treatment of complex patients.
* Assist with implementing exercise programs and progressing/maintaining physical ability to optimise mobility and independence for surgical, medical and long-stay inpatients, including those undergoing stem-cell transplants.
* Attend relevant ward MDT meetings, board rounds and handovers to identify new referrals and provide feedback on patient progress for physiotherapy and occupational therapy teams.
* Put in place post-op exercise and lymphoedema advice/information for surgical patients and refer patients to local wheelchair services as needed.
* Update electronic waiting lists and daily worklists to aid organisation and prioritisation of the team workload.
* Participate in audit and research projects within own area of responsibility.
* Recognise precautionary signs/symptoms and contraindications and seek supervision as required.
* Rotate with Band 4 between surgery and medical oncology wards as required.

* Record and monitor relevant patient data.
* Maintain excellent communication with patients, relatives and carers on treatment plans and progress.
* Demonstrate a variety of communication skills for patients with limited or restricted communication.
* Provide effective verbal or written feedback to therapists regarding patient care.
* Liaise with outside agencies and wider MDT members; participate in continual professional development.
* Maintain a personal development plan and participate in annual appraisal.
* Keep up to date with policies, procedures and protocols related to the Rehabilitation Unit and relevant clinical practice; attend staff meetings and mandatory training.
* Identify risk issues that impact patient rehabilitation and demonstrate high standards of clinical practice and teamwork.
* Be accountable for own practice and stay current with NHS practices.
Organisational
* Record and report systems related to service delivery (equipment, supplies, environmental issues).
* Relay confidential information as directed by qualified therapists.
* Use clinical databases (e.g., Careflow, electronic patient records, order systems).
* Participate in departmental projects and maintain up-to-date community referral information.
* Maintain stock of mobility aids, splints and braces and order items as requested by the physiotherapist.
* Keep the Rehabilitation/gym environment organized and clean; ensure information folders are up to date.
Qualifications
Essential
* Foundation degree in a health-related subject or equivalent (e.g., Diagnostic and Therapeutic Support).
Desirable
* EDCL or equivalent IT training.
Experience
Essential
* Worked in an Allied Health Profession or NHS setting.
Desirable
* Experience of working with adults in the physiotherapy role; experience in an Oncology setting.
