Role Overview

An exciting opportunity for a fixed term opportunity has arisen for a Band 7 Clinical Nurse Specialist to join the existing Malignancy of Unknown Origin/ Carcinoma of Unknown primary (CUP) Clinical Nurse Specialist team. This is a new and exciting role will be responsible for ensuring the delivery of high-quality care to patients who come under the remit of a suspected cancer with Non-Specific Symptoms referred to the center from the GP, patients presenting with a Malignancy of Unknown Origin and patients diagnosed with a Cancer of Unknown Primary. This role is a 6 month fixed term contract. We are looking for a committed, enthusiastic, Clinical Nurse Specialist, to provide keyworker support to these patients alongside developing and expanding an already excellent service. The post holder will be expected to contribute to the strategic direction and delivery of a quality service to patients and lead and develop this specialist nursing service within the Trust, working as an integral part of the multidisciplinary teams. The post holder will be part of the Oncology Service and responsible for ensuring the delivery of high-quality care to patients who come under the remit of a suspected cancer with Non-Specific Symptoms, Malignancy of Unknown Origin and Cancer of Unknown Primary.


Key responsibilities

* Co-ordinate the delivery of a comprehensive NSS, and MUO/CUP Service within the Trust.
* Work clinically and strategically with the local MDTs to monitor and develop policy, training and services against local and National guidelines including relevant IOG guidance and Cancer Measures.
* Provide Care and support to patients with non-specific but serious symptoms (NSS) who are referred into the service via the GP.
* Triage all incoming referrals from primary care, perform clinical assessment including degree of frailty, obtain full medical history and request appropriate investigations via Nurse led clinics (face to face or virtual).
* Provide Care and support to patients with a confirmed malignancy of unknown origin or Cancer of unknown Primary; presenting patients at NSS and CUP MDT.
* Work closely with Medicine, Oncology, surgery, clinical diagnostics and primary care to support the faster diagnosis and improve outcomes for NSS, MUO and CUP patients.
* Provide expert care and positively impact the experience of patients and carers by providing high standards of clinical practice.
* Support patients with suspected cancer facilitating necessary investigations and providing clinical engagement, providing advice and support.
* Manage a case load of patients who are under the NSS, CUP/MUO service.
* Undertake the role of key worker for the NSS patients and the MUO/CUP cancer patients.
* Facilitate the co‑ordination of patients through the MDT.
* Attend and present at weekly MDT's.
* Holistically assess patients at key areas of their journey.
* Sign post patients/carers/families to relevant support agencies.
* Order, assess and interpret diagnostic investigations within Trust policy and agreed pathways; use knowledge and skills to interpret results and implement care accordingly; refer to other specialised agencies when appropriate.
* Explain test results and consequences to patients, carers and staff, ensuring their understanding by careful and sensitive questioning.
* Be a resource both within the trust and the community for clinical advice and information.
* Support the professional development of students in liaison with the Practice Development team.
* Contribute to clinical governance within the trust for cancer patients.
* Deliver care in line with National and Alliance guidance and requirements.
* Provide nursing care through the provision of patient focused service, working autonomously.
* Triage referrals via the electronic referral system in line with the NSS referral guidelines.
* Deliver clinical care and management in line with local and national guidance and best practice.
* Put quality at the heart of practice by delivering evidence based individualised and personalised care through assessment, planning and evaluation of care interventions.
* Facilitate and support patient management, diagnosis and referral to specialist teams.
* Discuss investigations and treatment for cancer with sensitivity, knowledge and expertise.
* Demonstrate excellent communication skills and act as patient advocate when appropriate.
* Assess people's information needs and provide relevant information to meet those needs.
* Initiate and promote collaborative working relationships and effective communication between all members of the MDT to improve the management and outcomes of patients within the specialist areas.
* Provide specialist and advanced/expert advice and support to multi‑professional teams to improve care and support for patients with cancer.
* Act as an expert practitioner within the multi‑disciplinary team, providing specialist nursing knowledge, co‑ordinating specialised patient care and communicating changes in condition/treatment.
* Demonstrate advanced nursing practice in assessment, planning, implementation and evaluation of care and management of patients with cancer.
* Provide empathy and reassurance concerning quality of life issues associated with living with their disease and lifelong condition.
* Plan, deliver and evaluate programmes to address patients' complex health needs.
* Lead and manage a clinical case load for patients.
* Act as a non‑medical prescriber if required and determined by service need.
* Act as a role model and clinical expert within the trust and externally through demonstration of advanced knowledge, skills and evidence based practice.
* Maintain accurate records of all referred patients, ensuring relevant information and outcomes are documented in the patient's notes in accordance with the trust policy and professional standards.
* Provide ongoing care and support for patients with multi‑faceted/complex problems through one‑to‑one consultations as designated nurse‑led support.
* Use principles of health promotion and preventative strategies to provide education about healthy lifestyles to patients, carers, staff and the general public as appropriate.
* Ensure user involvement through the use of surveys, audit and attendance at relevant meetings.
* Promote the service and clinical excellence by publishing and presenting innovations, audit and research locally and nationally.
* Liaise and co‑ordinate with primary care, Trust colleagues, diagnostic services and the patient to ensure appropriate tests are completed and results are available prior to first clinical vetting/assessment.
* Work closely with the administrative services (including booking team), diagnostic services and clinical team to ensure next actions are regularly updated and completed.
* Act as professional supervisor and utilise clinical supervision and reflective practice skills to enhance development of self and colleagues in the inter‑professional team to optimise care.
* Act as a mentor for students and other junior staff, ensuring learning that is commensurate with education needs.
* Participate in nursing training and updates.
* Provide patients and their carers with information and advice, ensuring the best level of understanding as appropriate.
* Develop written and other health information for patients with regards to disease, treatment, management and ongoing care, specific to the NSS, CUP/MUO/ cancer pathway.
* Contribute to an annual training needs analysis to identify education and training needs of staff for improvement of patient care.
* Keep informed and update staff on developments in advanced/expert practice.
* Support the cancer nursing team as required.
* Seek opportunities to develop advanced/specialist nurse expertise to develop service and other skills.
* Be actively involved in the health promotion, education and identification of patients' needs, taking into consideration biological, behavioural, sociological and environmental factors.
* Actively support patients and their families, communicating difficult and complex information using appropriate and individualised methods to ensure effective understanding and empowerment for informed decisions.
* Continuously update and maintain own professional development in cancer through attendance at meetins/study days and utilisation of appropriate educational resources.
* Disseminate findings from own professional development through written reports, presentation and implementation of educational initiatives.
* Facilitate communication between primary, secondary and tertiary care providers across local Cancer services in the region.
* Develop evidence‑based protocols, policies and clinical guidelines for assessment and evaluation of impact on service delivery.
* Provide specialist advice and care with support and ability to work as an autonomous practitioner.
