Overview
These exciting posts will be based within the (Fife-wide) Integrated Discharge Team, mainly based within Victoria Hospital, Kirkcaldy and when required Community Hospitals across Fife.
These posts involve comprehensive assessment and discharge planning. We work to support discharge to meet PDD. Our ethos is patient centred pathways within the Home First Model. The post involves attending ward daily MDT’s completing assessments and communicating with the relevant teams across the Partnership and Third Sector organisations.
Responsibilities
* Carry out comprehensive assessment and discharge planning to support discharge to meet PDD.
* Attend ward daily multidisciplinary team (MDT) meetings, complete assessments and communicate with relevant teams across the Partnership and Third Sector organisations.
* Work across all areas and play a pivotal role in discharge planning with a dynamic approach to complex cases.
Qualifications and Skills
* Current qualification/registration in Nursing or an Allied Health Professional (AHP) role.
* Knowledge and wide range of experience in Health and Social Care is desirable.
* Excellent communication skills with an organised and flexible approach.
* Knowledge and experience of long-term conditions and frailty affecting the aging population.
* Autonomous practitioner capable of using own initiative, decision making, and providing regular updates on discharge pathways and plans.
* Proficiency in Microsoft Office and related databases (e.g., Excel).
