An exciting opportunity has become available for an enthusiastic and forward-thinking Health and Wellbeing Coach to join the K2 Grantham & Rural PCN. The successful candidate will be joining our Primary Care Community team, working with GPs, Nurses, Care Co-Ordinators, Occupational and Physiotherapists and Social Prescribing Link Workers. K2 Grantham Primary Care Network (PCN) is made up of the 10 practices situated in the South Kesteven District of Lincolnshire consisting of Colsterworth Medical Practice, Long Bennington Medical Centre, Market Cross Surgery, St John’s Medical Centre, St Peter's Hill Surgery, Swingbridge Surgery, Glenside Country Practice, Harrowby Lane Surgery, The Welby Practice, and Vine Street Surgery. The PCN serves a population of approximately 130,000 patients when combined with K2 Sleaford. Like all roles at Thrive Tribe, you can expect your time at work to be fun, varied and challenging. A typical day as a Health and Wellbeing Coach might include : A typical day as a Health and Wellbeing Coach might include : Using good communication, negotiation and people management skills with compassion and integrity. Working with a diverse range of people from different cultural and social backgrounds. Using coaching approaches, frameworks and models or other helping strategies, for example motivational interviewing, confidently and effectively in a varied and sometimes challenging environment. You will be working within the Primary Care Network to provide health coaching for the most vulnerable people in the community, including the frail / elderly and as well as patients living with long term health conditions (LTHC) or being identified as at risk of developing a LTHC, through current health and lifestyle behaviours. You will work closely with GP and practice teams to manage a caseload, ensuring appropriate support is available to people, families and carers. You will work alongside social prescribing link workers and care coordinators to provide an all-encompassing approach to personalised care and enable people to work out how best to use the health and care system. You will deliver sessions through a variety of mediums, including; face to face individual and group sessions, telephone appointments, online video calls and home visits where strictly necessary. It would be beneficial for the successful applicant to be able to work closely with practices and have greater visibility when supporting patients, so it is important that you are able to travel and can offer face to face appointments in practices where there is demand and space allows. Car use is essential for this role. You will work in a variety of settings including GP surgeries, Community Venues and remotely from home You will engage in partnership work with the GP practices to support referrals into your sessions. Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments – we’re always looking to share knowledge and resources!
