Band 7 Clinical Lead Nurse – All Age Continuing Care (AACC) service within North East and North Cumbria ICB
Main duties of the job
The role
As a Clinical Lead Nurse, you will:
* Provide day‑to‑day clinical leadership and specialist advice across the All Age Continuing Care service
* Support complex decision‑making, quality assurance, and consistency of eligibility and funding decisions
* Lead on complex case management, risk management, and safeguarding
* Provide professional supervision, mentorship, and support to nursing and multidisciplinary staff
* Contribute to service development, transformation, and continuous improvement
* Act as a key link with local authority partners, acute trusts, mental health services, and wider system partners
You will also support the service in meeting statutory responsibilities, managing performance, addressing backlog and risk, and embedding best practice across the region.
Job responsibilities
Lead a team of registered professionals to deliver the core functions of AACC. Provide expert clinical oversight of the application to the National Continuing Care Frameworks including eligibility decisions, disputes, appeals and PuPoCs. Where required support as a key leader in the delivery of national programmes such as Transforming Care and Inpatient Quality Transformation, LeDeR, S117 Aftercare, Neurorehabilitation and Complex Care. Where appropriate to the role ensure individuals are offered C(E)TRs in line with national guidance and within required timeframes, engaging with people with lived experience, their families, and carers to embed effective C(E)TR delivery within the local area. Attend MDTs and Predischarge meetings to support discharge for people with complex needs to ensure that they are supported appropriately in the appropriate placement that meets their needs. Participate in the operational aspects of the Learning from Lives and Deaths – People with a Learning Disability and Autistic People (LeDeR) programme and carry out LeDeR reviews as and when required, in accordance with nationally agreed standards and key performance indicators. Directly supervise the team in relation to Human Resources, e.g., absence management/sickness, appraisal and work performance. Ensure that assessments and reviews are carried out promptly according to national specifications.
Qualifications
* NMC or SWE Current Registration
* Degree masters level study in addition to post graduate diploma level specialist knowledge and/or equivalent experience.
* Valid UK driving licence.
Experience
* Extensive post registration experience, some of which will be at senior clinical level.
* Previous experience of working in a leadership role and the ability to plan and prioritise workload of the team.
* Extensive clinical experience of NHS Continuing Care.
* Experience of liaising with statutory and non‑statutory services and agencies.
* Research or audit experience.
* Previous experience communicating with senior management within the organisation.
* Experience working with Learning Disabilities or Mental Health.
* Experience of interpreting national and local policies related to All Age Continuing Care and case management.
* Experience of reviewing costs associated with care packages and making recommendations.
* Experience of ensuring compliance with documentation and accuracy of collation of information.
* Experience of participating in audit and service improvement.
* Experience of working autonomously within national policy and organisational policies.
* Community experience.
