We are looking for an Advanced Clinical Pracitioner to join our forward thinking and supportive team at Cauldwell Medical Centre (CMC) where you will be provided with an exciting opportunity to practice and further develop your clinical skills.
Main duties of the job
Main duties at Cauldwell Medical Centre include patient triage, home and nursing home visits, chronic disease management, acute assessments, and health promotion. The ACP supports QOF targets, reduces hospital admissions, and mentors nursing staff. They contribute to safeguarding, clinical supervision, and continuous service improvement, ensuring timely, safe, and holistic care across CMC.
1. The post holder will be an experienced clinician, who acting within their professional boundaries; will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice
2. The post holder will work collaboratively with the multi-disciplinary general practice team at CMC to meet the needs of patients, supporting the delivery of policy and procedures.
3. The post holder will build effective working relationships with other health care professionals.
4. Experience with Systmone
5. The post holder will work with the senior lead clinician and the other senior leads including lead GP and Practice Manager.
6. The post holder will work within the multi-disciplinary team to deliver excellent primary health care services within the practice.

12. Completed or working towards MSc ACP
13. The post holder will work within the multi-disciplinary team to deliver excellent primary health care services within the practice.

Essential criteria
14. Professional Registration
15. Post Graduate Qualification in specific specialty or equivalent experience
16. Advanced clinical Practitioner Master’s Degree
17. Independent Prescriber
18. Evidence of continuous learning and professional development
Desirable criteria
19. •Teaching and Mentorship (ENB 998 or equivalent)
Experience
Essential criteria
20. Significant recent experience in General Practice and primary care
21. Able to manage acute illnesses, long term conditions and minor ailments
22. Experienced and competencies in the management of Long term Conditions
23. Experience of working in a multidisciplinary way
Desirable criteria
24. Experience of leading on the delivery of QOF in General Practice
Knowledge and skills
Essential criteria
25. Specific knowledge of pertinent issues for Primary Care
26. Experience of Working with Homeless People / People with Challenging Behaviours
27. Advanced clinical skills and management of patients with complex issues
28. Excellent verbal and written communication skills with team members, patients, carers, and other healthcare professionals whilst recognising people’s needs for alternative methods of communication
