Overview
Community HCA, District Nursing Service, Alsager SMASH. Band 3 HCA role based in Sandbach within SMASH care community. Responsible for providing end of life care and a range of nursing interventions in patients\' homes as part of a busy community team.
You will work without close supervision, use your own initiative, and follow the patient\'s care plan. Excellent communication, empathy, and the ability to build strong relationships with patients and colleagues are essential. The post requires willingness to travel across the Central Cheshire Integrated Care Partnership footprint as needed.
Responsibilities
* Function as an effective member of the multi-disciplinary team
* Maintain clear, concise and legible documentation adhering to prescribed standards and protocols
* Use IT systems for relevant tasks (e.g., accessing patient records and results, data collection, inputting data)
* Obtain consent in accordance with Trust Consent Policy
* Report accidents/incidents on the clinical incident reporting system (domiciliary setting)
* Maintain a patient-centred approach to care delivery, promoting a rehabilitative and self-care approach
* Undertake phlebotomy duties as delegated and overseen by the registered nurse, following Trust policies
* Undertake clinical tasks as delegated by the Community Nurse team and within the sphere of competency
* Assist Community Nursing Team in management of patients with Long Term Conditions and End of Life care
* Recognise the need for referrals and liaise with other healthcare and multi-agency workers as necessary
* Participate in induction and orientation programmes for all staff requiring community experience as arranged by the Team Leader
* Maintain up-to-date knowledge of blood tests performed and ensure correct labelling of sample tubes at the time of sampling
* Maintain phlebotomy stocks, keeping equipment clean and in good order
* Follow Trust policy for needle-stick incidents and ensure accurate reporting on the appropriate system
