About Us: Our specialist Supported Living and Outreach service in Weymouth provides support for people with Learning Disabilities, Autism, Mental Health needs, and Acquired Brain Injuries. We work in partnership with Local Health Professionals such as social workers, community nurses, and specialists like Speech and Language Therapists, Physiotherapists, and Occupational Therapists.
Person Centred Planning
Our staff support each person to create a person-centred plan that details how they want to live their life now and in the future. Using a range of communication tools, we ensure each person is closely involved in developing their own plan.
Support Worker Responsibilities
* Providing day-to-day support and routine assistance.
* Promoting independence while helping individuals live fulfilling lives.
* Supporting and encouraging the development of personal skills through hobbies and interests.
* Maintaining and assisting with healthcare needs, including accompanying individuals to appointments or administering medication.
* Teaching life skills such as budgeting, paying bills, and shopping.
* Assisting with personal care as required.
* Helping with domestic tasks including cleaning and cooking.
* Offering guidance and emotional support.
Experience in the care sector is not essential, as we offer full training. However, we value enthusiasm and commitment to our company values to support people in living happy and healthy lives.
