The Central Supported Living Review Team is part of Integrated Adult Social Care, with strong links to Adult Community Health & Social Care Teams, commissioning, and market management. Currently focused on completing reviews in Supported Living placements, we work with people receiving statutory services from Adult Social Care across the Devon County Council footprint.
Our team comprises two Team Managers, Social Workers, and Social Care Assessors, working closely with Occupational Therapists and the Reaching for Independence Team in each locality.
The Central Supported Living Review Team fulfils the Local Authority’s duty under the Care Act 2014 to keep care and support plans under regular review, ensuring they remain appropriate, effective, and responsive to individuals’ changing needs, personal ambitions, and life choices.
We engage with people with disabilities to promote and maximise independent living skills, enabling individuals to connect with their community, gain employment, and live and enjoy their adult life as independently as possible - living the life that matters to them.

About You
As a Team Manager, your skills and experience will include:
• Experience working within social care, including in a multi-agency environment.
• Strong leadership and management skills, with the ability to effectively lead teams, develop and support individuals, and promote an environment of continuous improvement where everyone can achieve their potential.
• Ability to adapt and reprioritise as needed in a dynamic environment.
• Proven ability to build positive relationships and work collaboratively across teams and services.
• A commitment to our values of respect, integrity, and delivering high-quality outcomes for those we support.
• A relevant professional registration with a regulatory body.
• High-level written and verbal communication skills.
• Extensive knowledge and understanding of legislation, models, and theories relevant to Adult Social Care.
• Exceptional recording and planning skills.
• Ability to make difficult decisions confidently and appropriately.
• A valid UK driving licence and access to a car*
