Bury St Edmunds PCN has an opportunity for a care coordinator to join our primary care network (PCN), supporting five GP practices across the Bury area. The role is full-time, 37.5 hours per week, primarily on Monday and Friday between 08:30 and 18:00, with some weekend hours as needed.
The role involves working closely with practices, the multidisciplinary team (MDT), and the wider PCN to ensure patients receive optimal care. Responsibilities include working on requirements for the PCN Direct Enhanced Service Contract (DES).
Our care coordinators proactively identify and support vulnerable populations, including the frail/elderly and those with long-term conditions, providing care coordination and navigation across health and social care services.
Ideal candidates will have experience in care planning, strong communication skills, and excellent organizational abilities.
Main duties of the job
* Collaborate with patients, families, and carers to improve understanding of health conditions and develop personalized care plans.
* Assist patients in managing their needs by answering queries, managing appointments, and providing clear information to support decision-making.
* Work with GPs and primary care professionals to identify and manage patient caseloads, referring to other professionals as appropriate.
* Support multidisciplinary team coordination within the PCN.
* Promote shared decision-making and prepare staff and patients for these conversations.
* Develop communication channels among GPs, patients, families, carers, and other agencies.
* Maintain referral and intervention records for monitoring and evaluation.
* Assist practices in keeping care records current.
* Contribute to risk assessments and service evaluations.
* Collaborate with commissioners, locality teams, and agencies to develop the role further.
