Job Title and Summary

Job Title: Health Care Assistant (HCA)

Reports to: Practice Manager / Lead Nurse / GP Partner

Accountable to: Practice Manager

Location: Silverdale Medical Practice, Pendlebury Health Centre, 659 Bolton Road, Pendlebury, Manchester, M27 8HP

Hours: 20 hours per week

Job Summary: The Health Care Assistant (HCA) will work as part of the primary care team to deliver safe, effective, and high‑quality patient care. The post holder will undertake clinical tasks, support long‑term condition management, and contribute to health promotion activities, ensuring care is delivered in line with practice policies, safeguarding responsibilities, and patient safety standards.


Key Duties and Responsibilities


Clinical Duties

* Perform routine clinical observations including blood pressure monitoring, height, weight, BMI, ECG recordings, urinalysis, and phlebotomy (if trained).
* Undertake NHS Health Checks.
* Support chronic disease monitoring (e.g. diabetes, hypertension, COPD).
* Do home visits for patients who are housebound.
* Support immunisation and vaccination programmes.
* Carry out new patient health checks.


Patient Care

* Deliver high‑quality, patient‑centred care.
* Maintain dignity, privacy, and confidentiality at all times.
* Provide basic health promotion advice (e.g. smoking cessation, weight management, lifestyle).


Administrative Duties

* Maintain accurate patient records using clinical systems such as EMIS Web.
* Apply appropriate SNOMED coding where required.
* Maintain stock levels and order clinical supplies.
* Ensure clinical equipment is maintained and safe to use.


Safeguarding Responsibilities

* Demonstrate a clear understanding of safeguarding children and adults at risk.
* Comply with safeguarding policies and procedures at all times.
* Recognise signs of abuse, neglect, or exploitation and upscale concerns appropriately.
* Report safeguarding concerns promptly to the Safeguarding Lead.
* Maintain accurate and contemporaneous records of concerns.
* Participate in safeguarding training at the appropriate level for the role (minimum Level 2, or as required).
* Contribute to a culture of vigilance and patient safety within the practice.


Freedom to Speak Up (FTSU) Responsibilities

* Actively support a culture of openness, transparency, and learning.
* Raise concerns about patient safety, malpractice, or wrongdoing without fear of detriment.
* Understand and follow the practice’s Freedom to Speak Up policy.
* Escalate concerns through appropriate channels, including line manager, practice manager, and Freedom to Speak Up guardian if applicable.
* Support colleagues who raise concerns appropriately.
* Contribute to a positive safety culture aligned with NHS England FTSU principles.


Infection Prevention & Control

* Adhere to infection prevention and control policies.
* Maintain cleanliness of clinical areas.
* Safely handle clinical waste and sharps.


Professional Responsibilities

* Work within the defined scope of practice and competencies.
* Maintain mandatory training (including safeguarding, IPC, and basic life support).
* Participate in appraisal and supervision.
* Engage in audit, quality improvement, and service development.


General Responsibilities

* Participate in team meetings and training.
* Comply with all practice policies including health & safety, confidentiality & GDPR, equality & diversity.
* Work flexibly to meet service needs.

Person Specification


Qualifications

* NVQ Level 2 or 3 in Health and Social Care (or equivalent) or at least 2 years experience working in a Primary Care setting.
* Care Certificate (or willingness to complete).
* ECG training.
* NHS Health Check training.


Experience

* Experience working in a healthcare setting.
* Experience of direct patient care.
* Experience using clinical systems.
* Experience supporting long‑term conditions.
* Experience in General Practice.
* Knowledge of QOF or primary care performance frameworks.
* Experience supporting vaccination programmes.


Knowledge & Skills

* Understanding of basic clinical procedures.
* Knowledge of infection prevention and control.
* Awareness of safeguarding responsibilities and processes.
* Understanding of Freedom to Speak Up principles.
* Ability to use systems such as EMIS Web.
* Understanding of SNOMED coding.
* Awareness of quality improvement processes.
* Strong communication and interpersonal skills.
* Ability to work within a multidisciplinary team.
* Good organisational skills and attention to detail.
