Join Our Mental Health Social Care Leadership Team About the Team We are seeking a dynamic and experienced Service Manager to join our dedicated Mental Health Social Care Management Team. You will be part of a motivated group of professionals supporting the delivery of high-quality social care within an integrated health and social care setting. This includes oversight of our Urgent Care, Dementia, Older Adults, and Community Mental Health Team (CMHT) social care pathways, as well as our specialist Approved Mental Health Professionals and Mental Health Carers Service. About the Role This is an exciting opportunity for a confident and forward-thinking manager who thrives on challenge and innovation. As a Service Manager primarily supporting the Urgent Care pathway you will play a key role in strengthening our partnership with Coventry and Warwickshire Partnership Trust (CWPT) under our established Section 75 agreement. You will lead on embedding and enhancing our social work offer within Urgent Care Teams within mental health services including generic functions supporting the wider Community Mental Health Teams, ensuring a strong focus on Care Act responsibilities and person-centred outcomes for individuals experiencing acute mental ill-health and distress. Working closely with CWPT colleagues, and other partners you will: Support and develop effective systems for triaging and assessing urgent social care referrals Lead on improving the identification of and response to social care needs for individuals experiencing mental health crises. Monitor the quality of and report on the activity of social care within the mental health urgent care sector. Ensure and facilitate timely discharge planning for inpatients in mental health hospitals by ensuring early identification of social care needs and appropriate pathway access Oversee the delivery of a high quality Approved Mental Health Professional (AMHP) service which accords with national standards Produce annual reports on the activity of Warwickshire’s AMHP Service Play an active role in regional mental health and AMHP networks Work with partner agencies such as police, ambulance and urgent care teams to provide a joined-up system wide response to people experiencing mental health crises With Workforce Development identify mental health learning and development needs and commission training as appropriate. Engage in workforce planning to training, recruitment and retention of AMHPs If you are a qualified Approved Mental Health Professional (AMHP), you will also contribute to the countywide AMHP rota, supporting people of all ages in mental health crisis.
