This is an exciting opportunity to join our Home First Community Integrated Home First services. The team is expanding in response to our Health Board initiatives, which align hospital discharges to national discharge to recover and assess pathways and trusted assessor functions.

By bringing care closer to home and enhancing our community response the post holder will play a key role in the development of this discharge process, supporting acute care capacity and advocating ‘what matter’ to our patients.

The Home first nursing team aim to be innovative and person centred in the delivery of our service, enhancing quality and safety in the pathway and education of staff across organisations in the discharge process.

The post holder will provide clinical support and assessment working in partnership with health and social care teams across a wide range of settings, including domiciliary/therapy led care in the community. The team provide an in reach into our acute hospital to achieve a timely discharge into our step down beds or care home provision, with a ‘Home First Ethos’ as a trusted assessor.

Main duties of the job

Assess for all D2RA Pathways

Getting the patients to the right place at the right time,

Work collaboratively with other community professionals across the Region to assess services that provide clinical support and patient advocacy, Promote a Home First Ethos in all assessments conducted working with the community team to expedite release where appropriate.

Undertake the role of a trusted assessor for the identification and screening of appropriate patients for all D2RA pathways, Maintain optimal bed capacity and report/escalate any concerns to clinical leader/matron.

Support and report to Lead DLN activity and report to community leads on call all patient flow data, Support the clinical lead/Matron in the supervision, training and review needs of the H2H/Home first nursing team and the home care workforce.

Undertake clinical community roles and ensure a safe transition to longer-term or specialist community nursing services, Involve the patient/relative/carers in all aspects of recovery and enabling, Streamlining referrals through the D2RA (discharge-to-recovery and assessment) process with patient identification requiring nursing input.

Qualifications

Essential criteria

1. Registered Nurse.
2. Relevant Degree or experience
3. Demonstrate evidence of relevant post-registration education and professional updating.

Experience

Essential criteria

4. Clinical experience in Medical nursing.
5. Knowledge of discharge planning processes
6. Post registration experience
7. Experience at minimum Band 5
8. Can demonstrate a record of achievement in developing clinical practice. Can demonstrate an ability to manage others.
9. Participation in audit and education.
10. Knowledge of Clinical Governance.
11. Effective role model ,ability to utilize research and reliable evidence to inform and advance nursing practice.

Desirable criteria

12. Understanding of clinical and nonclinical risk assessment.
13. Knowledge of community services
